Responsibilities

This position coordinates all aspects of renal and pancreas transplants and supports the department's day-to-day functioning within the transplant section. The RN Transplant Coordinator functions as a case manager, incorporating all elements of Virginia Mason’s Nursing Professional Practice Model into the care of patients. This includes managing patients from the initial referral through the transplant evaluation process, during the inpatient hospital stay post-transplant, and during the transition back to the referring physician.

Job Summary:

The RN Transplant Coordinator plays a critical role in the Kidney and Pancreas Transplant Program, primarily responsible for one of three key functions: Pre-Transplant Recipient, Living Donor, or Post-Transplant Recipient. ** This particular position is for the Pre-Transplant Recipient Coordination role.  The Coordinator is expected to manage their assigned function while also supporting other areas as needed. This role involves comprehensive care coordination, patient education, and collaboration with multidisciplinary teams to ensure the smooth transition and ongoing care of transplant patients and living donors.

  1. Pre-Transplant Recipient Coordination:
  • Communicate with patients, referring physicians, and dialysis units regarding patient status within the transplant process.
  • Educate patients and caregivers across multiple states and time zones on the transplant process.
  • Coordinate all aspects of patient evaluations, including scheduling medical tests, follow-up appointments, and care coordination.
  • Collaborate with Virginia Mason providers, social workers, financial coordinators, and dialysis units to formulate comprehensive transplant plans and post-operative care strategies.
  1. Living Donor Coordination:
  • Manage pre- and post-donation activities, including patient and family communication, education, and coordination of medical testing and follow-up care.
  • Provide support in the operating room, including packaging kidneys for transplant.
  • Offer ongoing support and education to living donors throughout the donation process.
  1. Post-Transplant Recipient Coordination:
  • Conduct inpatient rounds and manage discharge planning for transplant patients.
  • Serve as a liaison between inpatient and outpatient areas, as well as other disciplines.
  • Perform post-operative assessments in the clinic to monitor patient progress.
  • Facilitate safe transitions of care for patients returning to their referring providers.
  • Triage patients throughout the post-transplant period to ensure long-term health and safety.

Additional Responsibilities:

  • Support patient and family experiences by providing multidisciplinary support groups both pre- and post-operatively.
  • Facilitate education classes for transplant recipients and their families.
  • Conduct outreach and education to referring providers and medical teams.
  • Ensure compliance with UNOS and CMS regulatory standards through meticulous practice and documentation.

Qualifications

A Washington State Registered Nursing License is required for this position.

Graduation from an Accredited school of Nursing.

BLS is required at time of hire and renewal every 2 years.

Ideal candidates will have the following: 

  • Understanding of End-Stage Renal Disease (ESRD): Comprehensive knowledge of the complexity of illnesses associated with ESRD, including risk factors, diagnostic, and treatment options.
  • Ability to Learn and Understand Organ Procurement and Distribution: Thorough understanding of the nationwide system of organ procurement and distribution, including knowledge of activating patients in the national registry and the acceptance procedures for organs.
  • Communication and Organization Skills: Excellent communication and organizational abilities are essential.
  • Computer Proficiency: Must be proficient with computers and relevant software.
