Assumes primary responsibility for overseeing, coordinating and facilitating comprehensive care delivery in all clinical areas during designated shifts. Assumes delegated nursing administrative responsibilities as directed.  Serves as the communication link between the care team and referring stakeholders. Participates in all activities, meetings, calls, to facilitate and prioritize bed assignments from all points of entry to the acute hospital setting. This position functions at the Division level and will coordinate bed placement activities across multiple hospitals. 


Responsibilities
    1. Receives bed placement requests, evaluates all available hospital beds and initiates, promotes and/or participates in interdisciplinary collaboration regarding patient placement; keeps abreast of bed demand/availability/status information in order to ensure patients are placed in the appropriate bed at the right time.
    2. Develops contingency patient placement plans with operational charge staff to ensure daily patient operations are not interrupted; continually re-evaluates requests as necessary to assure timely and appropriate bed assignments and optimal turnaround.  
    3. Monitors staffing and patient information to maintain real-time awareness patient demands, including patient location, acuity, risk factors, and turnaround times. 
    4. Maintains communication with appropriate stakeholders regarding bed availability or access delays; facilitates ongoing communication across work units regarding patient flow/bed placement status/issues.  
    5. Identifies anticipated patient admissions/transfers/discharges and ascertains potential bed availability on an organizational basis; maintains ongoing communication with the Emergency Departments, other requesting units and Transfer Center to identify shifting priorities/needs for beds.
    6. Gathers clinical information to clarify clinical acuity and other relevant information bearing on bed and/or hospital placement that is consistent with criteria for the patient’s needs.
    7. Serves as a resource to management/staff on clinical issues related to bed placement.
    8. Assures that barriers to timely bed placement are addressed and resolved to promote both customer and patient satisfaction.
    9. Monitors unusual patient placement situations, admission problems or complaints and collaborates with appropriate staff on the patient care unit.
    10. Fosters a collaborative professional relationship with patient logistics team, hospital departments/managers, physicians and other stakeholders to assure best practice is the focus for all patient-centered quality care.
    11. Maintains awareness of campus bed statuses (available, clean, dirty, blocked for maintenance, etc.)
  • Utilizes resources to identify and aide in expediting patient care transitions (such as discharge/release) facilitate


Qualifications

Bachelor’s Degree in Nursing preferred and 3 years clinical experience required
